Annual Forms
These forms are needed by every student every year- We prefer you fill out a hardcopy of these forms provided by school on a specific color of paper to help our staff quickly access needed information in files.
Release Authorization- List of individuals who are authorized to pick-up students from school
Emergency Medical Authorization- Enables parents/guardians to authorize the provision of emergency treatment for children who become ill or injured while under school authority, when guardians cannot be reached.
As Needed Annual Forms
Child Medical Statement- Required for yearly for 3-6 students and once for elementary students. Bring to your child’s doctor to complete and attach vaccination records. Submit to school.
Request for Administration of Medication- Please print and fill out for for medication to be administered at school. Medication must be in original packaging. Prescriptions must be in prescription bottles. (Pharmacies will provide an extra bottle when requested.) Over the counter medications must have a doctor signature on this form. All medication must be handled only by adults.
Physical Care Plan- (for chronic medical issues like asthma, severe allergies, etc.)
Permission Slip for Sunblock, Insect Repellent, Lotion, and Chapstick
Carseat Exemption Permission Slip Optional for ages 9 and Older
